LPN (0-3) - Shift 1

Job Purpose:

The Licensed Practical Nurse (LPN) at

LIFESTREAM BEHAVIORAL CENTER

will provide compassionate and professional nursing care to patients in a behavioral health setting. The role involves working closely with a multidisciplinary team to support the well-being and recovery of individuals experiencing mental health challenges. This position is for Shift 1 and is ideal for candidates with 0-3 years of experience in nursing.

Key Responsibilities:

Administer prescribed medications and treatments in accordance with nursing standards and protocols. Monitor patients' health status and report any changes or concerns to the healthcare team. Assist in the development and implementation of individualized care plans for patients. Maintain accurate and timely documentation of patient care and progress. Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care. Provide education and support to patients and their families regarding health management and treatment plans. Uphold the highest standards of patient confidentiality and ethical practice. Participate in ongoing training and professional development opportunities to enhance nursing skills and knowledge. Ensure a safe and therapeutic environment for all patients and staff members.

Required Education:

Graduate from an accredited Licensed Practical Nurse (LPN) program.

Required Experience:

0-3 years of experience as a Licensed Practical Nurse, preferably in a behavioral health or similar healthcare setting. Experience in providing direct patient care and managing patient records.

Required Skills and Abilities:

Strong understanding of nursing practices and procedures. Ability to work effectively in a team environment and collaborate with other healthcare professionals. Excellent communication and interpersonal skills to interact with patients, families, and staff. Proficient in using healthcare software and electronic medical records. Ability to handle stressful situations and respond to emergencies calmly and efficiently. Strong organizational skills and attention to detail in maintaining patient records and documentation.
