  • Part-time, Full-time Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our assisted living and memory care team.
The ideal candidate will provide high-quality patient care in assisted living and memory care communities. This role requires a strong understanding of nursing practices, excellent communication skills, and the ability to work collaboratively with healthcare professionals to ensure optimal patient outcomes. Duties Administer medications and treatments as prescribed by physicians. Conduct patient assessments and monitor vital signs. Assist in the development and implementation of patient care plans. Document patient information accurately in MAR systems. Collaborate with interdisciplinary teams to coordinate patient care. Educate patients and families on health management and disease prevention. Support patients with activities of daily living (ADLs) and mobility assistance. Ensure compliance with HIPAA regulations and infection control protocols. Qualifications Valid LPN license in the state of practice. Experience in assisted living and memory care preferred. Experience working with diverse populations including individuals with developmental disabilities, Alzheimer's care, memory care, and geriatrics is highly desirable. Strong skills in patient assessment. Excellent communication skills for effective interaction with patients, families, and healthcare teams.
