Job Description

Job Summary Join our dedicated adult day care team as a Licensed Practical Nurse (LPN), where your expertise will directly enhance the quality of life for our senior and adult clients. In this vital role, you will provide compassionate, comprehensive nursing care tailored to individuals with diverse needs, including those with dementia, Alzheimer's, and other memory impairments. Your energetic approach will ensure a safe, engaging environment that promotes health, independence, and well-being. As an integral part of our team, you will coordinate care plans, monitor health status, and support daily activities with a focus on safety and dignity. Responsibilities Conduct thorough patient assessments, including vital signs, medication administration, and physical examinations to ensure optimal health status. Administer medications and treatments such as injections, IV infusions, catheter care, and wound management in accordance with physician orders and safety protocols. Monitor patients for signs of acute pain or changes in condition, utilizing triage skills to prioritize urgent needs and communicate effectively with healthcare providers. Maintain accurate medical documentation within electronic health record (EHR) systems while ensuring compliance with HIPAA and long-term care regulations. Assist with activities of daily living (ADLs), including mobility support, tube feeding, and personal hygiene to promote patient comfort and safety. Collaborate with interdisciplinary teams to develop individualized care plans that address cognitive impairments like dementia or behavioral health challenges. Educate patients and their families on health management strategies, medication adherence, and safety measures for memory care and geriatrics. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the state of employment. Demonstrated experience in adult or long-term care settings such as skilled nursing facilities, assisted living communities, or hospice environments. Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques relevant to geriatrics and memory care. Experience administering IV infusions, injections (including venipuncture), catheterization, and managing complex medication regimens. Familiarity with infection control practices and acute care procedures including airway management and pain management protocols. Ability to work effectively with individuals with developmental disabilities or behavioral health needs; experience in Alzheimer's or dementia care is highly valued. Strong critical thinking skills for triage assessments and emergency response within fast-paced healthcare environments such as hospital or emergency room settings. Excellent communication skills to collaborate with multidisciplinary teams while maintaining clinical confidentiality policies under HIPAA standards.
