In this vital role, you will provide high-quality, patient-centered nursing care directly in patients' homes, promoting health, independence, and well-being. Your expertise will help deliver personalized care plans, monitor patient progress, and support families through complex health needs. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring ongoing health support. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and medication reconciliation to develop tailored care plans. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Manage wound care, catheterization, tube feeding, and ventilator management to ensure optimal patient comfort and safety. Document all patient interactions accurately using electronic health record (EHR) systems such as Epic or Cerner while maintaining HIPAA compliance. Provide education on disease management, medication adherence, and lifestyle modifications to patients and their families. Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to specialists such as infectious disease or dementia care providers. Respond promptly to triage calls and urgent needs by assessing symptoms and determining appropriate interventions or escalation procedures. Skills Proven experience in critical care settings such as ICU or trauma centers with knowledge of Level I and Level II trauma protocols. Strong background working with individuals with developmental disabilities, Alzheimer's disease, dementia, memory care needs, or behavioral health challenges. Proficiency in medical documentation using EMR/EHR systems like Athenahealth or eClinicalWorks; familiarity with
ICD-9/ICD-10
coding is a plus. Skilled in performing invasive procedures including phlebotomy, venipuncture, catheter care, sterile processing techniques, and sterile technique practices. Knowledge of advanced patient assessment techniques including physiology knowledge and diagnostic evaluation skills. Ability to manage acute pain cases and provide emergency interventions such as airway management or spinal tap when necessary. Experience with home & community care environments including assisted living facilities or post-acute care settings; familiarity with hospice or palliative care is advantageous. Excellent communication skills for working with children, seniors, and individuals with disabilities; capable of providing health coaching and education tailored to diverse populations.
