• Job Description:

• + Demonstrates ability to request care from referring sources within the community in a timely manner.

+ Reviews patient medical records for Performance Improvement/Risk Management per established criteria and document adverse patient occurrences. + Collects information and documents patient and family reasons for readmission. + Documents discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs according to standard work. + Provides patient and family advocacy, and support patient's choice and patient rights during hospitalization.
