Full Job Description Professional Solutions Home Health Agency is seeking qualified per diem/full time/part time LPNs Licensed Practical Nurses in the area of Broward County.

IMMEDIATE POSITIONS AVIALABLE.

Responsabilities Responsible for the delivery and coordination of quality patient care in compliance with physician orders. Continuously observes and assesses patient condition and care needs and reports changes in condition to the supervisor and/or physician as appropriate. Documents all activities, assessments, nursing actions, responses and coordination of care in a timely manner whenever care is delivered. Takes appropriate nursing action based on assessment and achieves expected outcomes. Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk. Educates the patient and family regarding the disease process, self-care techniques, and prevention strategies, and in meeting the patient's nursing needs. Specific requirements for this role include:

• Diploma, Associate' s or Bachelor's degree in nursing from a state accredited program
• Current and unrestricted LPN license to practice in the state of Florida Current CPR certification
• Current Continuing Education as required by state
• Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
• Proof of eligibility to work in the United States
• Ability to work independently and with minimal supervision
• Adult home care background preferred
• Excellent people, verbal and written communication skills
• Current vehicle liability insurance
• Bilingual preferred, not required
