Job Summary Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) in a Skilled Nursing Facility (SNF), where your expertise will directly impact patient care and recovery. This role offers all shift options—day, evening, and overnight—allowing flexibility to fit your lifestyle. As an essential member of our care team, you will provide compassionate, high-quality nursing services to residents, ensuring their comfort, safety, and well-being. Your skills in patient assessment, medication administration, and clinical procedures will help create a nurturing environment for individuals with diverse health needs. Responsibilities Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation. Administer medications, injections, IV infusions, and other treatments following physician orders and safety protocols. Monitor patients' conditions closely, recognizing signs of deterioration or acute pain requiring immediate intervention. Assist with complex procedures such as catheterization, tube feeding, wound care, and sterile techniques. Maintain accurate medical records using electronic health record (EHR) system like PointClickCare (PCC); document all patient interactions thoroughly. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident's needs. Ensure compliance with HIPAA regulations and infection control standards while safeguarding patient confidentiality. Experience Valid Licensed Practical Nurse (LPN) license in good standing with the state licensing board. Demonstrated proficiency in medical documentation systems including EMR/EHR platforms like PCC.
