Position Overview The Licensed Practical Nurse (LPN) provides direct nursing care, medication administration, resident assessments, care coordination, and clinical oversight within our Assisted Living and Memory Care community. This position plays a vital role in ensuring residents receive safe, compassionate, and personalized care while maintaining compliance with state regulations and community policies.

Executive Director Key Responsibilities:
Resident Care & Nursing Services Provide quality nursing care and support to residents in Assisted Living and Memory Care. Administer medications and treatments as prescribed. Conduct ongoing resident assessments and monitor changes in condition. Document observations, incidents, interventions, and care provided accurately and timely. Assist with the development and implementation of individualized service plans. Promote resident health, comfort, independence, and emotional well-being. Provide assistance with activities of daily living (ADLs) when needed. Respond appropriately to medical emergencies and resident concerns. Clinical Oversight & Leadership Provide shift supervision and clinical guidance to Care Associates and direct care staff. Assist with training, mentoring, and supporting team members. Ensure resident care is delivered according to established standards and care plans. Communicate effectively with physicians, families, pharmacy providers, and interdisciplinary team members. Participate in quality assurance and performance improvement initiatives. Compliance & Safety Maintain compliance with federal, state, and local regulations. Follow infection prevention and control procedures. Ensure accurate documentation and medication records. Participate in emergency preparedness, fire safety, and disaster response procedures. Support a safe environment for residents, families, visitors, and staff. Qualifications Required Qualifications Current and active Licensed Practical Nurse (LPN) license. Knowledge of medication administration, pharmacology, and nursing best practices. Strong clinical assessment and critical thinking skills. Excellent communication, documentation, and organizational skills. Ability to work effectively in a team-oriented healthcare environment. Preferred Qualifications Experience in Assisted Living, Senior Living, Long-Term Care, Skilled Nursing, Rehabilitation, Home Health, or Memory Care. Experience caring for residents with Alzheimer's disease or other forms of dementia. Previous leadership, charge nurse, or supervisory experience. CPR/First Aid certification (if required by state regulations). Physical Requirements Ability to stand, walk, bend, reach, and move throughout the workday. Ability to assist residents with mobility, transfers, and activities of daily living. Ability to lift, push, and/or pull up to 50 pounds.
