Job Summary Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) specializing in private duty nursing. In this vital role, you will provide personalized, high-quality nursing care directly to patients in their homes or private settings. Your expertise will help improve patient outcomes, promote independence, and support families through compassionate and skilled care. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring specialized medical attention, including those with complex health conditions and disabilities. Duties Deliver comprehensive nursing care tailored to each patient's needs, including medication administration, wound care, IV infusion, and catheterization. Perform detailed patient assessments, vital sign monitoring, and physical examinations to inform treatment plans. Manage advanced medical procedures such as ventilator support, tube feeding, and sterile techniques for infection control. Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with physicians, case managers, and family members to develop and implement effective discharge planning and ongoing care strategies. Conduct triage assessments over the phone or in person to determine urgency and appropriate interventions for patients with acute or chronic conditions. Provide specialized care for individuals with dementia, Alzheimer's disease, developmental disabilities, or those requiring memory care or hospice services. Qualifications Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience in critical care settings such as ICU, emergency room, trauma centers (Level I & II), or medical-surgical units. Strong background in complex medical procedures including IV infusion, medication administration, phlebotomy, and catheter care. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation. Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques. Experience working with diverse populations including pediatrics (neonatal ventilator), geriatrics (long-term care), and individuals with disabilities or behavioral health needs. Ability to perform physical assessments and vital signs efficiently while demonstrating excellent patient care skills. Certifications in Basic Life Support (BLS) and sterile processing are preferred; additional certifications like PICU experience or hospice care are a plus.
