MDS Nurse (RN/LPN) Ayers Health & Rehabilitation Center 606 NE 7th St, Trenton, FL 32693 Full-Time | Skilled Nursing Facility Bring Your MDS Expertise Where It Truly Matters Ayers Health & Rehabilitation Center is seeking an experienced MDS Nurse (RN or LPN) to join our clinical team. This role is ideal for a nurse with hands-on experience completing both skilled (Medicare) and long-term care MDS assessments, with a strong understanding of wound-related coding and documentation. We are looking for someone who is detail-oriented, clinically knowledgeable, and committed to ensuring accuracy, compliance, and optimal reimbursement—all while supporting positive resident outcomes. Work Today, Get Paid Today - Daily Pay Available Competitive pay based on experience Medical, Dental, Vision, and 401(k) Paid Time Off + Holidays Company-paid life insurance Tuition reimbursement & CE opportunities Supportive leadership team and collaborative environment Key Responsibilities Complete and submit accurate and timely MDS assessments (MDS 3.0) for both skilled (Medicare) and long-term care residents Set, track, and manage Assessment Reference Dates (ARDs) Ensure documentation supports clinical conditions, including wounds and complex care needs Conduct and complete Care Area Assessments (CAAs) Collaborate with interdisciplinary team members on care planning Review clinical documentation to ensure accuracy, compliance, and reimbursement optimization Maintain up-to-date knowledge of Medicare/Medicaid guidelines and regulatory requirements Support survey readiness and participate in audits as needed Qualifications (Required for Consideration) Active RN or LPN license in the state of Florida (required) Proven experience completing MDS assessments in a skilled nursing or long-term care setting (required) Experience completing skilled (Medicare) MDS assessments (required) Experience with long-term care residents and wound-related MDS coding/documentation (required) Strong knowledge of: MDS 3.0 process RAI (Resident Assessment Instrument) Manual Care Area Assessments (CAAs) Understanding of Medicare/Medicaid reimbursement guidelines Strong attention to detail and organizational skills Preferred Experience 1-2+ years of experience in a skilled nursing facility (SNF/LTC) Experience with wound care, rehab, or case management Familiarity with electronic health record systems What We're Looking For A detail-driven nurse who understands the impact of MDS accuracy Someone who can work independently while collaborating with the care team A professional who takes ownership of timelines, compliance, and quality Disclaimer This job description is not intended to be all-inclusive.
