LPN (Licensed Practical Nurse)

Overview A Home Health Agency is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. This role requires a strong commitment to patient well-being, effective communication skills, and the ability to work collaboratively with other healthcare professionals. Duties As a Licensed Practical Nurse, your responsibilities will include: Assisting in the assessment of patients' health status and needs. Administering medications and treatments as prescribed by physicians. Monitoring patients' vital signs and reporting any changes to the healthcare team. Providing primary care support, including acute pain management and assistance with daily living activities. Collaborating with healthcare professionals to develop and implement patient care plans. Educating patients and their families about health conditions, medications, and self-care strategies. Utilizing medical imaging techniques as needed for patient assessments. Maintaining accurate patient records and documentation in compliance with healthcare regulations. Experience Candidates should possess the following skills and experience: Valid LPN license in the state of practice.
