GENERAL DESCRIPTION OF POSITION

The Assisted Living LPN provides person centered care to the patients/residents. They are responsible for the delivery of care to the residents. They administer medication as ordered, respond to the resident's health needs, supervise and oversee the CNAs in the delivery of care and communicate with families/designated contacts as needed. Attend care plan meetings as needed. Follow facility policies and procedures as well as infection control policies. Essential Duties & Responsibilities for a Assisted Living Licensed Practical Nurse (LPN): Administers medications/treatments as ordered. Communicates information regarding resident's condition with MD and healthcare team and resident representative. Documents all necessary information accurately and thoroughly. Supervises CNAs and oversees basic care. Completes all other duties as directed by Assisted Living Administrator. Participates in care plans as needed.
