We are seeking a dedicated Licensed Practical Nurse (LPN) to join our home health team in the Albany, Georgia area. In this role, you will manage your own routine of patient visits, delivering high-quality skilled nursing care directly to patients in the comfort of their homes. Because we serve a broad community, this position is ideal for an autonomous nurse who enjoys the open road— visits span up to a 45-mile radius from Albany. Key Responsibilities Conduct routine home health visits to administer prescribed medications, wound care, injections, and other skilled nursing interventions. Monitor, record, and report changes in patient conditions to the Registered Nurse (RN) Case Manager or physician. Educate patients and their families on care plans, medication management, and disease prevention. Maintain accurate, timely clinical documentation in compliance with state and federal regulations. Travel efficiently to patient homes within a designated 45-mile radius.

Compensation & Schedule Benefits Per Visit Rate:

$26.00 per completed visit.

Travel Support:

$0.50 per mile reimbursement to offset your travel and fuel costs.

Requirements License:

Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Georgia.

Experience:

Minimum of 1 year of clinical nursing experience (Home Health or Public Health experience is highly preferred).

Travel:

Must have a valid driver's license, reliable transportation, and active auto insurance, with the willingness to drive within a 45-mile radius.

Skills:

Strong clinical judgment, excellent communication skills, and proficiency with electronic electronic medical records (EMR) charting.

Certifications:

Current CPR/BLS certification.

Pay:

Up to $26.00 per hour
