Job Description

Right at Home is hiring for an LPN! We provide non-medical home care to the geriatric population. As an LPN Home Care Coordinator, you will play a crucial role in instructing our caregivers how to provide compassionate and comprehensive care to our clients in their homes. Working closely with our team of caregivers, you will assess client needs, develop care plans, and ensure the highest standard of care delivery. LPN Home Care Coordinator requires strong clinical skills, excellent communication, and a genuine commitment to improving the lives of others. Monday through Friday, 9a-5p. Competitive starting salary!
Responsibilities of LPN Home Care Coordinator:
Conduct comprehensive assessments of clients' health status and needs. Develop personalized care plans in collaboration with the client, family, and Right at Home team. Monitor and evaluate clients' progress and adjust care plans as necessary. Educate clients and their families on health management. Coordinate care with other healthcare professionals, community resources, and family caregivers. Maintain accurate and up-to-date documentation of all care provided. Ensure compliance with regulatory standards and company policies and procedures. Participate in ongoing training and professional development activities. Foster a positive and supportive environment for clients, families, and team members. Oversee and train caregivers on proper techniques for providing non-medical care, including personal hygiene, mobility assistance, and companionship. Provide guidance and support to caregivers in managing challenging client situations and implementing care plans effectively. Conduct regular performance evaluations and provide feedback to caregivers to ensure quality care delivery and adherence to company standards.
Qualifications of LPN Home Care Coordinator:
Current LPN license in GA. Previous experience in home care or long-term care preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Compassionate and empathetic attitude toward clients and their families. Reliable transportation and valid driver's license. Clear background check and drug screening.
