Job Description

We are seeking a compassionate, skilled, and dependable Licensed Practical Nurse (LPN) to provide one-on-one, in-home nursing care to an individual enrolled in the Georgia DBHDD (Department of Behavioral Health and Developmental Disabilities) waiver program. In this role, you will work directly inside the individual's family home, delivering physician-ordered clinical care, implementing behavioral support plans, and helping the individual maintain independence, safety, and a high quality of life in their community.

Key ResponsibilitiesSkilled Clinical Care Administer prescribed medications, treatments, and therapies according to the physician's Plan of Care. Perform specialized nursing tasks which may include G-tube management, tracheostomy care, wound care, bowel regimens, or oxygen therapy. Monitor, record, and report changes in the individual's vital signs, physical health, and mental status to the supervising RN. Assist with physical transitions, positioning, and mobility utilizing proper body mechanics and assistive transfer devices (e.g., Hoyer lifts). Behavioral Support & Client Safety Implement and follow the individual's positive Behavioral Support Plan (BSP) to manage and de-escalate challenging behaviors. Maintain a calm, safe, and structured environment that minimizes triggers and promotes emotional well-being. Provide crisis intervention if behavioral health emergencies arise, following approved safety protocols. Family Collaboration & Daily Living Support Partner closely with family members and primary caregivers to ensure seamless continuity of care. Respect the privacy, culture, and dynamics of the family's home environment. Assist with activities of daily living (ADLs) directly related to the individual's medical condition, such as specialized feeding, personal hygiene, and grooming. Documentation & Compliance Complete accurate, real-time documentation of daily electronic nursing notes, medication administration records (MAR), and behavioral tracking logs. Maintain strict confidentiality in compliance with HIPAA guidelines. Adhere to all Georgia Medicaid, DBHDD, and agency compliance and safety standard

$30.00 - $33.00 per hour
