Primary Function:
To assist the Tender Loving Care Private Home Care team in providing nursing care to Private Home Care patients.
Supervision:
Clinical Nurse Manager Qualification:
1. Current Georgia license as a Licensed Practical Nurse. A minimum of 2-3 years nursing experience desirable. Ability to work well within an interdisciplinary team setting as well as independently. Ability to work a flexible schedule. Has never been shown by credible evidence to have abused, neglected, assaulted, exploited, or deprived any person or to have subject any person to serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at time of application.
Training:
Orientation to Private Home Care services and any other training that may be necessary to complete the assigned duties effectively.
Duties:
An LPN assigned to a patient/family has responsibilities for assessing in their needs and for developing, implementing, and evaluating a plan of nursing care. In addition, the LPN provides direct patient care on an ongoing basis, in accordance with the Nurse Practice Act and Private Home Care Standards. The LPN shall do the following: Assist with implementing a plan of care, with goals defined by the patient/family and the team. Provide nursing services according to the plan of care that has developed. Instruct the family on Private Home Care techniques. Report any pertinent changes in patient's condition to the Clinical Nurse Manager. Making nursing services available on an ongoing basis. Make regular home visits as ordered by MD and as needed. Assist in providing 24-hour on-call 7 days a week. Serve as a member of the Tender Loving Care Private Home Care team. Performs other duties as assigned by the
