  • In Home Care professional, where you will play a vital role in providing compassionate, comprehensive nursing services directly in patients' homes.
This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring specialized care, including those with chronic conditions, disabilities, or recovering from illness. You will deliver personalized patient care, monitor health status, and support families while ensuring compliance with healthcare standards and regulations. Your expertise will help promote independence and improve quality of life for your patients in a comfortable home environment.
Duties:
Perform initial and ongoing nursing assessments Monitor vital signs including blood pressure, pulse, respiration, and temperature to track patient health trends Perform specialized nursing procedures such as catheter care, wound management Administer medications and treatments as ordered by the Physician and document accordingly Maintain detailed medical documentation adhering to HIPAA regulations and ensure proper management of medical records Support patients with dementia or Alzheimer's by implementing memory care strategies and providing compassionate companionship Use medical terminology confidently to communicate effectively with healthcare teams and family members Operate medical equipment safely and competently while adhering to all safety protocolsExperience Proven experience in home health nursing environments with a focus on senior care or assisted living settings Hands-on experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease Knowledge of medical terminology, medication administration procedures, and vital signs monitoring Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as catheter care are a plus Ability to work independently while managing multiple patient cases effectively in a home setting Strong communication skills to collaborate with healthcare teams, patients, and families confidently Educate PCAs on the client's Plan of Care. Document clinical notes within 24 hours of the shift.
RN Only:
Supervise PCAs/LPNs and perform 60-day supervisory visits.
Requirements:
