Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, and acute care environments. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, thorough assessments, and personalized care plans. Responsibilities Deliver comprehensive nursing care by performing physical examinations, vital signs monitoring, and patient assessments to determine needs and develop appropriate care plans. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and safety protocols. Manage IV infusions, catheterizations, tube feedings, and ventilator support for patients requiring specialized interventions. Document all patient interactions thoroughly using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems such as Epic, Cerner, or Athenahealth. Assist with diagnostic evaluations including specimen collection, processing, and medical imaging procedures when necessary. Provide wound care, suture removal, and aseptic techniques to prevent infection and promote healing. Collaborate with multidisciplinary teams on discharge planning, case management, and ensuring continuity of care across inpatient and outpatient settings. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in the designated area. Proven experience working in diverse healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, hospice care, or outpatient clinics. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and medication administration protocols. Experience with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation and patient record management. Demonstrated skills in patient assessment, triage procedures, pain management—including acute pain—and emergency response techniques. Familiarity with specialized care areas such as Alzheimer's/dementia/memory care, infectious disease management, pediatric or neonatal care is preferred. Ability to work effectively with individuals with developmental disabilities or those requiring complex medical interventions like ventilator support or spinal taps. Excellent communication skills combined with a compassionate approach to patient-centered care; physical ability to stand for extended periods and perform lifting or repositioning tasks safely.
