Job Description

Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, efficient community base nursing support in private homecare settings. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to determine individual care needs. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management. Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing to ensure patient safety and comfort. Utilize electronic health record (EHR) systems like Epic, Cerner, or Athenahealth for precise documentation of patient care activities and medical records. Support patients with complex conditions such as dementia, Alzheimer's disease, or those requiring hospice or palliative care by providing specialized memory care and behavioral health support. Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or home & community care services. Conduct patient education on health coaching topics including nutrition, medication administration, infection control, and chronic disease management. Manage airway devices such as ventilators and perform diagnostic evaluations like laboratory specimen collection or sonography when needed. Maintain strict adherence to HIPAA regulations to protect patient confidentiality and ensure compliance with healthcare standards. Demonstrate proficiency in medical terminology, anatomy knowledge, ICD coding (ICD-9/10), CPT coding, and other clinical documentation requirements. Experience Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in Basic Life Support (BLS). Prior experience working in acute care settings such as hospitals or Level I/II trauma centers is highly desirable. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Experience in specialized areas including ICU, emergency medicine, medical-surgical units, geriatrics, or pediatrics is preferred. Knowledge of infection control practices, sterile techniques, and aseptic procedures essential for safe patient care. Background working with individuals with disabilities or developmental delays enhances your ability to provide personalized support. Familiarity with advanced nursing procedures such as IV insertion/infusion therapy, venipuncture, phlebotomy, and catheter care is advantageous.
