Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and comprehensive assessments. Duties Deliver direct patient care by performing physical examinations, vital sign assessments, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA regulations and infection control standards. Assist with complex procedures including IV infusion, catheterization, tube feeding, wound suturing, and sterile dressing changes utilizing aseptic techniques. Conduct patient triage in urgent care or emergency settings, evaluating symptoms related to acute pain management, infectious diseases, or trauma cases in Level I or Level II trauma centers. Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care coordination to ensure seamless patient transitions. Support specialized care needs such as Alzheimer's care, dementia management, hospice services, or pediatric and neonatal patient assessments. Document all patient interactions thoroughly in medical records using EMR/EHR systems like Athenahealth or eClinicalWorks to ensure accurate medical documentation and compliance. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare environments including hospitals (ICU experience preferred), nursing homes, outpatient clinics, or home care settings. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with
ICD-9/10
coding is a plus. Critical care experience or background in emergency medicine, ICU nursing, or trauma centers highly desirable. Knowledge of physiology and anatomy fundamentals along with skills in patient assessment and diagnostic evaluation. Ability to perform phlebotomy, venipuncture, medication administration via injections or IV insertion with precision and safety. Strong understanding of infection control practices and HIPAA compliance to protect patient confidentiality at all times. Excellent communication skills for working with individuals of all ages—including children with special needs—and collaborating effectively within healthcare teams.
