Job Description

Overview Join our dedicated healthcare team as a Licensed Practical Nurse (LPN), where your expertise will make a meaningful difference in patient care across diverse settings. In this vital role, you will provide compassionate, high-quality nursing services, supporting patient recovery and well-being. Your skills in medical documentation, patient assessment, and medication administration will ensure safe and effective care delivery. Responsibilities Perform comprehensive physical examinations and patient assessments to determine individual needs and develop personalized care plans. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Monitor vital signs, manage airway equipment such as ventilators, and provide acute pain management for patients with complex conditions. Document all patient interactions accurately using electronic health record (EHR) systems like Epic, Cerner, or Athenahealth, ensuring compliance with HIPAA regulations. Assist with diagnostic evaluations, specimen collection/processing, and wound suturing to support physicians' assessments. Provide specialized care for patients with dementia, Alzheimer's disease, or those requiring memory care or hospice services. Collaborate with interdisciplinary teams including physicians, case managers, physical therapists, and social workers to coordinate comprehensive patient care. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Demonstrated experience in various healthcare environments such as hospitals (including ICU, emergency room, trauma centers), nursing homes, assisted living facilities, or outpatient clinics. Proficiency in EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with
ICD-9/ICD-10
coding is highly desirable. Experience in G-J tube care is preferred. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology to perform accurate patient assessments and interventions. Experience working with individuals with developmental disabilities or those requiring specialized dementia or memory care is preferred. Ability to handle complex cases involving acute pain management, infection control, ventilator management, catheterization, tube feeding, and sterile techniques. Excellent communication skills for triage assessments and patient education; comfort working in fast-paced emergency medicine or urgent care settings is a plus. $32.00 - $35.00 per hour
