Job Description
What You'll Do - Your Purpose in Action Champion resident wellness by identifying and responding to evolving care needs, ensuring residents receive the highest quality of personalized service. Conduct timely assessments at movein, 30day, semiannual, and statuschange intervals in partnership with the Health & Wellness Director (HWD). Lead monthly wellness visits and monitor clinical status, vitals, weights, and medication profiles with care and accuracy. Respond to incidents such as falls or changes in condition with proper evaluation, documentation, and followup. Maintain accurate wellness documentation and ensure compliance with Thrive policies and regulatory standards. Communicate effectively with residents, families, and healthcare providers to ensure collaborative care planning. Support clinical compliance and infection control protocols, ensuring safety and wellness in every aspect of care delivery. Promote a residentcentered care model, quality care outcomes, and continuous improvement through active participation in clinical meetings and quality assurance initiatives. Assist team members with clinical support and lead by example in professionalism, compassion, and skill.
