Elderly Disabled Waiver Program (EDWP) and incident reporting. Assist the Nurse Manager with changes in the Individual's Care Plans. LPN is able to sign off on the Individual's Care Plan if no changes are made that relates to medical care. Communicate any concerns of the Individual with the Case Manager, Behavioral Specialist, Support Coordinator and/or the Individual's support team. Complete supervisory visit for Individual via in home or phone communication: at least every two months (60-62 days) or every 90 days, notifying the RN of any significant change. Coordinate and provide skilled nursing and routine care procedures for the Individual as needed under doctor instructions and orders. Document observations and complete progress notes of the Individual's condition during and after each visit. Provide watchful oversight, support, report and document changes in medical condition of all individuals in the Community Residential Alternative homes. Review Individual Service Plan tracking of objectives and create a monthly report.
SUMMARY:
Licensed Practical Nurse oversees the care and needs of the Individual following the physician orders in the Community Residential Alternative Home and/or the private residence.SKILL COMPETENCIES

Duties and responsibilities may include but are not limited to the following: Monitor and record Individual's vital signs as needed. Observe and report changes in the Individual's physical condition to the Manager Nurse. Review and monitor Individual's charting in the Generations Homecare Systems. Conduct training and education to employees. Update Health Risk System Tool (HRST) as needed Follow doctor's orders for Individual's medication and/or services. Enter progress notes and supervisory visit entries, documenting the Individual's Health status. Accompany Individuals with virtual doctor appointments and schedule appointments as needed. Communicate with the nurse manager and program coordinator with physician feedback related to medical appointment. Educate the Individual, family and caregiver for the Individual's care to include:medication, special equipment, diagnosis, and care plan. Review, sign, date and record supported services when provided, entries include:Elderly Disabled Waiver Program (EDWP) and incident reporting. OTHER DUTIES AND RESPONSIBILITIES AS ASSIGNED
Dress, redress wounds and assess the healing process. Other duties as required to maintain the overall operation of Family Bridge Healthcare.QUALIFICATIONS
A current Georgia Nursing License. Must possess a current, valid LPN license in state of practice A minimum of two years' experience in home health services or a related field.working with aging population is preferred. Clear DBHDD or GCHEXS criminal background check. Ability to supervise caregivers. Proficient reading and writing skills, verbal and written communication skills. Knowledge of current regulations, Compliance, Federal and State laws, and Department of Behavioral Health and Developmental Disabilities (DBHDD) programs, and other government waiver programs such as CCSP, SOURCE, NOW, COMP, ICWP and Structure Family Caregiver. Knowledge of clinical skills including but not limited to changing bandages, feeding, administering medication by injection or mouth, and insulin monitoring. Ability to meet deadlines. Make independent decisions when required. Filing, telephone communication, faxing, shredding, and correspond with emails daily Intermediate computer skills including the ability to prepare correspondence and reports.Job Type:
