Job Description
Job Summary Join our dedicated team as a Pediatric Private Duty Nurse (LPN/RN) providing in-home, personalized care for children with complex medical needs. You will be responsible for assessing patient conditions, administering treatments, and collaborating closely with families and healthcare teams to ensure optimal health outcomes. Responsibilities Deliver comprehensive nursing care to pediatric patients in their homes, including medication administration, IV infusions, tube feedings, and wound care. Conduct thorough patient assessments, vital signs monitoring, and physical examinations to evaluate health status and identify changes requiring intervention. Manage complex medical procedures such as catheterization, ventilator management, and sterile dressing changes using aseptic techniques. Maintain accurate medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Educate families and caregivers on medication administration, disease management (including Alzheimer's and dementia care), and safety protocols to promote independence and well-being. Collaborate with physicians, case managers, and other healthcare professionals to develop and update individualized care plans tailored to each child's needs. Respond promptly to urgent situations through triage skills, providing critical interventions for acute pain management or infectious disease concerns while ensuring safety at all times. Skills Proven experience with Epic EMR systems or similar EHR platforms such as Athenahealth or eClinicalWorks. Critical care experience including ICU or Level I Trauma Center background is highly desirable. Strong knowledge of pediatric physiology, anatomy, and diagnostic evaluation techniques. Skilled in IV insertion, phlebotomy, medication administration including injections, and catheter care. Familiarity with medical terminology related to pediatrics, neurology (e.g., neonatal ventilator management), and chronic conditions like cerebral palsy or developmental disabilities. Experience working with children across various settings such as hospitals, nursing homes, assisted living facilities or home & community care environments. Ability to perform comprehensive patient assessments utilizing vital signs measurement and physical examinations effectively. Knowledge of infection control practices including sterile processing and aseptic technique. Capable of managing complex cases involving pain management, gastrostomy tube feeding, or post-surgical care such as suturing or spinal taps. Excellent communication skills for teaching families about health coaching strategies while adhering to HIPAA standards.
