Key Responsibilities:

Deliver skilled nursing care to home health patients according to physician orders and individualized care plans Assist the RN Case Manager with patient assessments, care coordination, and treatment implementation Perform wound care, IV infusions, PICC line management, tube feedings, and catheter changes Document patient care accurately using Homecare Homebase (HCHB) and OASIS systems Educate patients and families on medications, treatments, disease management, and self-care techniques Maintain infection control and OSHA safety standards during all patient visits Monitor patient conditions and report changes to the RN Case Manager and physician Assist with patient admissions, transfers, and discharges Participate in interdisciplinary case conferences and team communication Provide compassionate end-of-life and hospice support when required Maintain productivity standards of approximately 30 units weekly and 7-8 patient visits daily

Qualifications:

Current LPN/LVN license in the State of Georgia required Minimum 2 years of LPN/LVN clinical experience required At least 1 year of recent Home Health experience required Homecare Homebase (HCHB) experience required OASIS documentation experience required Current CPR certification required Valid driver's license, reliable transportation, and current auto insurance required Strong skills in wound care, IV therapy, PICC lines, tube feeding, and catheter care preferred Excellent communication, time management, and patient care skills required

Pay:

$1,500.00 - $1,600.00 per week
