Job Description

Position Summary Utilizing the Nursing Process of assessing, planning, implementing and evaluating, the Licensed Practical Nurse is responsible for the delivery of patient care for adult and geriatric clients, and promoting collaboration among the multi-disciplinary health care team. Maintains open communications and is available to component organizations and practitioner sites for consultation. Education College Diploma in Nursing
  • Required Required to be a graduate from an accredited nursing program as a Licensed Practical Nurse Experience 1 Year acute hospital setting preferred
  • Required Community Health Nursing preferred but not mandatory. License & Certification Professional License with State of Practice
  • Required Basic Life Support (BLS)
  • Required Core Job Functions Follows and implements physician ordered interventions.
Patient Care Coordinator or medical personnel are notified immediately of potential patient problems and situations. Assess and interpret data in order to identify each patient's status and needs and provide the appropriate care. Participates in the on-call process. Records information accurately and legibly on appropriate forms or in computer devices according to agency policies and procedures. Collaborates with all personnel through patient team conference to identify, clarify and respond to the needs of the patient. Agency policies and procedures are followed in the care, maintenance and use of all supplies and computer hardware. Maintains reliable transportation and ability to travel as required. Information recorded accurately and legibly on appropriate forms or in computer devices. Assessment, implementation, resolution, planned goals and nursing care are documented. Completes all required documentation. Assess patient's personal abilities and limitations and requests assistance in situations that exceed limitations utilizing supervisor and others in authority appropriately.
