Job Description
Primary Purpose:
Provide health care oversight, direct nursing care, and coordination of medical services to individuals with intellectual and developmental disabilities residing in a CRA, CLS, Day Program, ensuring compliance with DBHDD, HFRA, and state regulatory requirements.Core Job Duties:
Health Care Oversight & Coordination Monitor, assess, and document individuals' health status regularly. Develop and implement individualized healthcare plans in collaboration with interdisciplinary teams. Ensure timely scheduling and follow-up for all medical, dental, vision, and specialist appointments. Communicate with physicians, pharmacies, and other medical providers to coordinate care. Direct Nursing Care Administer prescribed medications and treatments per physician orders. Perform nursing procedures (e.g., wound care, tube feeding, catheter care) as required. Provide first aid and emergency care when necessary. Monitor and document vital signs and health-related data. Medication Management Oversee medication administration system, including MAR (Medication Administration Record) maintenance. Conduct medication counts, audits, and ensure proper storage of medications. Train and oversee Direct Support Professionals (DSPs) in proxy caregiver medication administration (if applicable). Ensure medication error reporting and corrective action. Health & Safety Compliance Ensure compliance with DBHDD Health & Safety standards , HRFA rules , and Individual Service Plans (ISP) requirements. Participate in internal and external audits, site reviews, and inspections (e.g., DBHDD, HFRA). Maintain accurate and up-to-date nursing and medical records. Training & Support Train and support CRA staff on health-related topics (e.g., seizure management, infection control, diabetes care, nutrition). Provide education to individuals and families on health maintenance and wellness. Participate in interdisciplinary team meetings and contribute to care planning. Documentation & Reporting Complete required documentation timely and accurately (nursing notes, incident reports, health assessments). Track and report communicable diseases and other reportable conditions as required. Maintain confidentiality in compliance with HIPAA standards. Quality Assurance & Continuous Improvement Participate in Quality Assurance (QA) activities, health risk screening tool (HRST) monitoring, and follow-up. Implement health protocols and monitor their effectiveness. Assist in investigations of healthcare-related incidents and implement corrective actions. Additional Duties Participate in on-call rotation for medical support as needed. Provide support during emergencies, evacuations, and drills. Other duties as assigned by the Nursing Supervisor or Program Director. Qualifications (Typically Required) Licensed Practical Nurse (LPN) in Georgia. Experience working with individuals with intellectual and developmental disabilities (preferred). Knowledge of DBHDD, HFRA, and Georgia proxy caregiver regulations (preferred). CPR and First Aid certification (required).Pay:
