Job Description

Job Overview We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our healthcare team on the first shift, with no weekend requirements. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs. Administer medications and injections accurately following established protocols, including IV infusions and sterile techniques. Document all patient interactions thoroughly using electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's condition and preferences. Manage acute pain and chronic conditions through appropriate interventions, including wound care, catheterization, and medication management. Provide specialized care for patients with dementia or Alzheimer's disease in memory or assisted living settings, ensuring safety and comfort. Assist with medical procedures such as tube feeding, venipuncture, suturing, and spinal taps while maintaining aseptic techniques. Support discharge planning and case management efforts to ensure smooth transitions across care settings. Skills Proficiency in EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks for accurate documentation and communication. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods. Experience working in diverse clinical environments including long-term care facilities, hospitals (Level I/II Trauma Centers), emergency rooms, or outpatient clinics. Skilled in patient assessment techniques including vital signs monitoring and physical examinations. Expertise in medication administration protocols including injections, IV insertion/infusion, and catheter care. Familiarity with ICD coding (ICD-9/10) for accurate medical records documentation. Ability to work effectively with individuals with disabilities or developmental challenges in various settings such as home & community care or assisted living. Knowledge of infection control practices and sterile processing procedures to ensure patient safety.
