Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic home care team. In this vital role, you will provide high-quality nursing care directly in patients' homes, promoting health, safety, and independence. Your expertise will support individuals across diverse age groups and health conditions, ensuring they receive personalized, comprehensive care in a familiar environment. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a flexible and rewarding setting. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Manage complex wound care, including dressing changes and infection control measures, especially for patients with chronic wounds or post-surgical needs. Assist with activities of daily living (ADLs), such as mobility support, catheterization, tube feeding, and personal hygiene to enhance patient comfort and safety. Coordinate with physicians, case managers, and other healthcare professionals to develop and implement individualized care plans aligned with patient goals. Educate patients and their families on health management strategies, medication adherence, and disease processes like Alzheimer's disease or dementia care. Maintain accurate medical documentation in compliance with HIPAA regulations and ensure timely reporting of patient progress or concerns. Skills Proven experience with Epic EMR systems or similar EHR platforms such as eClinicalWorks or Athenahealth for documentation and patient management. Critical care experience including ICU or emergency medicine settings is highly desirable for managing acute situations effectively. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Skilled in IV insertion, catheterization, phlebotomy, medication administration, and sterile technique procedures. Experience working with individuals with developmental disabilities or those requiring specialized memory care such as Alzheimer's or dementia care. Ability to perform patient assessments accurately while demonstrating excellent communication skills for triage and health coaching. Familiarity with home & community care standards, post-acute care protocols, hospice care practices, and infection control procedures.
