JOB DESCRIPTION
SUMMARYOF POSITION A
Behavior Health Nurse- LPN provides nursing care to chronically or acutely sick patients; provides medication monitoring, adjusts medications within limits prescribed by Medical Director or designate, and performs in liaison role between client and Medical Director or designate, lab, pharmacist, family or residential facility staff, and other community caregivers; provides information, education and support for clients; promotes wellness, recovery and skill building to sustaining better heath; prevention training in the areas of wellness, recovery, managing symptoms, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provides life skills and wellness didactic services utilizing evidence-based methodologies to provide excellent clinical services. Prepare for and conduct individual, group, and family therapy for assigned caseload. (for clinical/direct service jobs). Assist service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered. Monitor service recipient progress in treatment and participate in treatment plan reviews as assigned. Update treatment plans as appropriate. Submit treatment plans at all stages for medical review, if applicable. (clinical/direct service) Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conduct assessment and write assessment summaries, as assigned. (clinical/direct service) Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services. And maintain appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate. (clinical/direct service) Assist service recipient in making discharge plans and provide follow up medical services as assigned (clinical/direct service). Demonstrates an ability to manage crisis situations, including danger to self and others, to ensure the safety and stabilization of consumers and to make appropriate referrals/linkages when clinically indicated. Does not leave crisis situation until consumer is in a safe environment or crisis is resolved. (clinical direct service) Attend staff meetings, clinical staffings, and other internal meetings as assigned. Participate on committees as assigned, including quality assurance functions. Maintain appropriate communication with other employees and work as an effective member of treatment team. Administers and gives medications to patients, monitors the dosage, side effects, use and documents patients' reactions. Provides medication training to the client or client's family or guardian to administer the client's medication, to monitor proper levels of dosage and watch for side effects. Obtains laboratory specimens and help doctors with patient examination and continuity of care arrangements with other providers Monitor urinalysis, breathalyzer, and other testing as assigned.KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods. Knowledge of community geographical awareness and of community and agency resources. Skill in customer service techniques.QUALIFICATIONS
Education Level Completion of LPN program. Years of Experience N/A Certification/Licensure Must meet Qualified Mental Health Professional status in relevant programs. Active unencumbered Illinois LPN license.PHYSICAL REQUIREMENTS
