Pay:

$60-$120/visit Acuity Home Health is looking for compassionate RNs to provide skilled nursing care in patients' homes. Join our team and help patients recover, stay independent, and live healthier lives.

What You'll Do:

Deliver high-quality care based on individualized patient plans Monitor conditions, administer medications, and perform treatments Educate patients and families on health, prevention, and safety Document care accurately in EMR Collaborate with doctors, nurses, LPNs, and CNAs Supervise LPNs/CNAs and participate in ongoing training

Qualifications:

Illinois RN license in good standing Home health experience / OASIS knowledge is a plus Proficiency in WellSky Kinnser EMR is a plus CPR certification Reliable vehicle & valid driver's license Comfortable using smartphone/EMR for charting
