• Manage and deliver patient care plans collaboratively within a team
• Provide wound care services to patients in the coverage area
• Maintain accurate and timely charting and documentation
• Use EMR software and mobile devices for patient care management
• Drive to patient locations within the assigned coverage area Schedule and Shift Details
• Monday - Friday, 8:30 AM - 5:00 PM; no weekends, holidays, or on-call; typically sees 6-7 patients per day Travel
• Must live within 30 minutes of coverage area; approximately 20% driving to patient locations Qualifications Required Associate degree or higher 1+ years of experience in ANY of the following: Home Health Care Home Health 1+ years of experience in ANY of the following: Electronic Medical Record (EMR) Other EMR Practice Partner EMR Valid Licensed Practical Nurse (LPN) in Illinois (IL) Valid Certified Wound Ostomy and Continence Nurse (CWOCN) Valid Regular Driver License (DL) in Illinois (IL) Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?
