Responsibilities:

Provide individualized care to residents, developing and implementing personalized care plans in collaboration with our multidisciplinary team Conduct initial and ongoing assessments of residents' conditions, identifying changes and making adjustments to care plans as needed Collaborate with our medical director and care team to monitor and update resident progress, communicating with physicians and families to ensure seamless care Set discharge dates and plans, coordinating with outside services to ensure a smooth transition back home Participate in multi-disciplinary team meetings to discuss resident care and develop strategies for improved outcomes

Requirements:

Current LPN or RN licensure in the state of Illinois 0+ years of experience in a healthcare setting (education and training also considered) Ability to work part-time hours, with flexibility to adjust schedule as needed
