This role offers an exciting opportunity to provide comprehensive patient care across diverse clinical settings, utilizing advanced medical knowledge and a compassionate approach. The ideal candidate will possess extensive clinical experience, including working with specialized populations such as pediatric and geriatric patients. This position emphasizes delivering high-quality care through collaboration, innovation, and adherence to best practices in healthcare. Duties Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to inform diagnostic and treatment plans. Manage acute and chronic conditions across multiple specialties such as internal medicine, geriatrics, pediatrics and behavioral health. Utilize electronic health record (EHR) systems such as Practice Fusion for accurate documentation of patient encounters and medical records management. Provide patient education on medication administration, disease management, nutrition, and health coaching tailored to individual needs. Support telehealth initiatives by conducting virtual consultations while maintaining HIPAA compliance and high standards of patient confidentiality. Qualifications Valid licensure as a Nurse Practitioner or Physician Assistant with current certification from an accredited body. Familiarity with CPT coding and ICD coding (ICD-10). Strong clinical skills Excellent communication skills for triage assessments, patient education, case management, and interdisciplinary collaboration.
