This role offers a highly consistent overnight schedule and the opportunity to make a direct impact supporting residents living with memory loss.

You will:

Conduct resident assessments and support individualized care plans Administer medications and treatments accurately Monitor resident conditions and communicate changes appropriately Ensure proper documentation of care and services Support and guide caregiving staff during overnight shifts Promote a calm, safe, and supportive environment for memory care residents Help residents maintain dignity, comfort, and quality of life Core Responsibilities Medication administration and treatment management Resident monitoring and clinical support during NOC hours Accurate and timely documentation Collaboration with care team and leadership Adherence to all state regulations and clinical standards What We're Looking For Active LPN license in Illinois Experience in memory care, assisted living, or long-term care preferred Strong clinical judgment and attention to detail Ability to work independently during overnight shifts Compassionate, patient, and team-oriented approach Reliable and committed to consistent scheduling
