  • Full-time Marion County Horizon Center is seeking a Full-Time Licensed Practical Nurse (LPN) to provide daytime nursing support (Monday-Friday) for adults with intellectual and developmental disabilities (IDD) in a CILA (Community Integrated Living Arrangement) setting in Rantoul, IL.
This position is clinically focused and works in partnership with a Residential House Manager and RN-Trainer (RN-T) to ensure individuals receive high-quality, coordinated care. This home supports individuals with complex medical needs, making this an ideal role for an LPN who wants to fully utilize their clinical skillset in a community-based setting.
Position Overview:
Schedule:
Monday-Friday Key Responsibilities (Clinical Focus): Coordinate, schedule, and attend medical appointments Monitor overall health status and respond to changes in condition Provide care for individuals with G-Tubes, ostomies, and specialized diets Assess and treat skin issues, wounds, and ongoing medical concerns Maintain detailed medical documentation and MARs Communicate with physicians, specialists, pharmacies, and care teams Implement and monitor individualized health care plans
Qualifications:
Current Illinois Licensed Practical Nurse (LPN) license (required) Valid Driver License Experience in long-term care, developmental disabilities, or high-acuity residential settings preferred Hands-on experience with wound care, G-Tubes, ostomy care, and chronic condition management strongly preferred Strong assessment, documentation, and communication skills Ability to work independently while collaborating with an interdisciplinary team Basic computer proficiency (EMR, Microsoft Office, Google Suite)
