60013, 61063, 60152 (Crystal Lake, Roscoe, Marengo & surrounding communities) Compassionate and detail-oriented Home Health LPN with 2+ years of experience providing skilled nursing care in home settings across designated ZIP code territories. Experienced in supporting RN case managers, delivering direct patient care, and ensuring continuity of care for patients with complex medical needs.

Key Responsibilities:

Provide skilled nursing visits under RN supervision across ZIP codes 60013, 61063, and 60152 Perform focused assessments and report changes in patient condition to supervising RN/physician Deliver hands-on care including wound care, Foley catheter care, ostomy care, and basic PICC line maintenance per agency protocol Administer medications as ordered and reinforce medication adherence Assist with monitoring of chronic conditions such as CHF, COPD, diabetes, and post-hospital recovery Educate patients and caregivers on disease processes, safety, infection prevention, and daily care needs Document patient care accurately and timely in EMR according to agency and Medicare guidelines Collaborate with interdisciplinary team including RN, PT/OT, and home health aides
