Tendercare Home Health is seeking dedicated and compassionate Private Duty LPNs to join our growing team, serving Indianapolis and the surrounding areas. Responsibilities Include:
Follow individualized care plans and document patient progress. Ability to use critical thinking to assess, plan, and implement care in a home environment. Recognize and report changes in client's condition to the Case Manager and Physician in a timely manner. Provide direct patient care, including but not limited to, medication administration and vital sign monitoring, physical assessments and treatments. Accurately complete both electronic and paper documentation in compliance with state and federal guidelines. Promote personal safety and maintain a safe environment for clients, including adherence to infection control protocols. Communicate effectively and professionally with patients, families, and members of the healthcare team. Perform physical tasks, such as lifting, repositioning, and providing mobility assistance to ensure patient comfort and safety. Work independently to deliver compassionate patient-centered care that promotes dignity and emotional well-being in a respectful home environment. Participate in continuing education as required.
Required Qualifications Include:
LPN Graduate of an accredited School of Nursing Active Indiana LPN license in good standing Current CPR certification Strong written and verbal communication skills Knowledge of OSHA, HIPAA, and FDA standards. Candidate must be able to lift 50 pounds and pass a fit-to-work screen Scheduling Options (Depending on patient options): Days Nights Weekends Part Time Full Time Weekends, Nights and Holidays are NOT required but available to those wanting other shifts
Pay Range:
