About the Role The Licensed Practical Nurse (LPN) Home Health position provides compassionate and skilled nursing care to residents in their homes. This role focuses on delivering personalized resident care plans, monitoring health status, and assisting with medical treatments to promote recovery and maintain quality of life. The LPN will collaborate closely with healthcare teams, residents, and families to ensure effective communication and continuity of care. Emphasis is placed on resident safety, adherence to clinical protocols, and timely documentation of care activities. Ultimately, this role aims to support residents' independence and well-being while managing their health needs in a home setting. Job Responsibilities Provide direct nursing care to residents in their homes, including administering medications, wound care, and monitoring vital signs. Communicate with residents, families, and healthcare providers by answering phones and coordinating care plans. Document all resident interactions, treatments, and observations accurately in compliance with healthcare regulations. Educate residents and their families on health management, medication adherence, and lifestyle modifications to support recovery. Minimum Qualifications Valid Licensed Practical Nurse (LPN) license in the state of Indiana. Certification in CPR and First Aid. Experience in resident care, preferably in a home health or clinical setting. Strong communication skills, including proficiency in answering phones and basic patient interaction. Preferred Qualifications Previous home health nursing experience. Additional certifications related to chronic disease management or wound care. Familiarity with electronic health record (EHR) systems. Demonstrated ability to work independently and manage time effectively in a home care environment.
