Full Job Description Qualifications:
Must be a graduate of an accredited school of nursing Must have current state Licensed Practical Nurse license Must have current CPR/First Aid certification Must complete provider orientation and patient care orientation Must have knowledge of I/DD regulations and the interdisciplinary team process Must have a valid driver's license, reliable vehicle and be insurable under company policy Must have the ability to handle issues resulting in positive outcomes Must have knowledge of the needs of persons with I/DD and associated syndromes Must have the ability to interact autonomously, respectfully and efficiently Must have the ability to work with interdisciplinary team members and family members of those receiving services Must have a professional appearance, strong organizational and communication skills Must be detail orientated Must have skills in computer applications including Word, Excel, Internet and E-Mail (Office 365) Must have the willingness to leave office to work with individuals being served in a positive and meaningful way Must be able to bend, stoop, push, pull reach, sit, and walk for periods of time. Must be able to utilize proper body mechanics while lifting up to 75 pounds.
Job Summary:
Endless Abilities, Inc. has in place for its services a Licensed Practical Nurse. The Licensed Practical Nurse will, in accordance with established policy, be responsible to the legal nursing entity for ensuring all areas of Endless Abilities, Inc.'s standards are met. The Licensed Practical Nurse will work directly under the Director of Nursing and will be responsible to report to the Corporate Compliance Committee. Duties and Responsibilities include, but are not limited to: Assist with recruitment, direction and performance of staff Assist with the organization of the administrative functions of the program, delegation of duties and establishing a formal means of accountability on the part of subordinates Provide supervision and administrative support to the individuals and employees of Endless Abilities, Inc. by implementing and monitoring operation policies and procedures Required to follow federal, state and regulatory guidelines and accreditation standards Ensure quality of supports and services provided to individuals we serve Ensure compliance with policies and procedures of Endless Abilities, Inc. Conduct regular visits to homes ensuring compliance with all Endless Abilities, Inc. procedures and policies as well as State and Federal guidelines with Wellness Coordination regulations Complete documentation of Wellness Visits/Consultations within 24 hours of completion Notify the Director of Nursing of any significant findings, concerns, or change in health status during the Wellness Assessments/Consultations Complete Wellness Coordination tracking log and turn it in to the Director of Nursing by the 5th of each new month Liaison to families and individual of those being served by Endless Abilities, Inc. Liaison to other service agencies being utilized by Endless Abilities, Inc. Assist the Director of Nursing with training Assist the Director of Nursing with the following: Emergency room visits Medication orders Entering/removing order from eMAR (Maintaining eMAR records) Medication audits Scheduling appointments Medication exchange Obtaining/returning meds from pharmacy Assessing drugs interactions/side effects Contesting medical bills Resource management. (HHC, Hospice, equipment, Diabetic supplies, etc.) On-call during designated on-call days Diabetes training House specific medical training (vitals, g-tube, catheters, breathing treatments, injections, etc.) Skin assessments/nail care/Wound care. Scheduled IM injections Inspecting medical equipment Coordinating doctor visits/appointments Incident reporting Collaboration with specialties
Training:
Core A/B, Change in status, new equipment, etc. Auditing appointments schedules (450Bs, Psych, vision, etc.) Medication/Specialty training Hospital visits, continuity of care, discharge planning Nurse visits/Wellness visits Implementing systems to reduce Medication errors Assessing, planning, implementing, & evaluating systems of care delivery Participating in conferences with physicians, hospitals and reps from other agencies Collaborating and updating case managers, guardians and physicians Participating in quarterly meetings when requested to do so Other duties as assigned
