NURSE, LPN/RN JOB QUALIFICATIONS & SUMMARY
Qualifications:
  • Minimum Licensed Practical Nurse (LPN) with former experience in a home care setting
  • Hold a current State License
  • Knowledge and application of Windows software and E-mail
  • Be able to attend training as required by L.I.F.E., Inc.
  • Knowledge of MR/DD regulations and the interdisciplinary team process
  • Have a valid driver's license and be insurable under company policy
  • Ability to handle issues resulting in positive outcomes
  • Knowledge of the needs of persons with disabilities
  • Ability to interact appropriately, respectfully and effectively
  • Ability to work with significant others and family members of those receiving services
  • Professional appearance, strong organizational and communication skills
  • Be detail orientated
  • Willingness to leave office to work with individuals being served in a positive and meaningful way
  • Skills in computer applications including Word, Excel, Internet and E-Mail.
  • Good oral and written communication skills
Job Summary:
L.I.F.E., Inc. has in place for its services a Nurse. The Nurse will, in accordance with established policy, be responsible to the legal entity for ensuring all areas of L.I.F.E., Inc.'s standards are met. The Nurse will provide optimum services to individuals receiving services in order to enable them to achieve daily medical and nutritional needs. The Nurse will be responsible for maintaining medical documentation and follow up on all assigned individuals that meet the state regulatory requirements for the Program Service. The Nurse works under and reports directly to the Director of Nursing. Duties and Responsibilities include, but are not limited to:
  • Maintain current knowledge of all federal, state and accreditation regulations as they pertain to clinical services of to clients and ensure correct application of such services
  • Conduct and perform the requirements of Health Care Coordination as specified in the client care plan
  • Conduct routine visits to client residences to monitor medication regime, medication administration, treatments and review of medical documentation contained in the individual Service Records in the home
  • Responsible for the training of staff in the implementation of treatments, use and care of adaptive/assistive devices, proper medical documentation and the administration of medications
  • Conduct updated training of medical documentation and medication administration as regulations change
  • Assist with maintaining the medical section of client main files, ensuring that they are current and contain the required documentation
  • Coordinate each client's medical and dental appointments, physician's orders and related medical services
  • Establish and maintain contact with physicians, psychiatrists, dentist, pharmacists and other care providers in order to provide quality medical care to each client
  • Serve as interdisciplinary team member. Communicate client health care needs to the appropriate Case Manager, Service Coordinator, Program Director, etc.
  • Conduct quarterly clinical assessments on all clients as required
  • Complete
MOSES/DISCUS
assessments of clients prescribed psychotropic medications at the time of the individual's quarterly nursing assessment
  • Responsible for achieving the required annual Physical exams, annual Mantoux or Chest x-rays, Dental, Vision and Hearing evaluations along with the documentation and outcomes of such examinations
  • Complete assessments and medical staffing reports on new clients as required and set up the Medical Card System on each at this time
  • Develop an Initial Health Care Plan for each consumer and review and make corrections as necessary
  • Complete case conference medical staffing reports as required
  • Complete follow up on reported medical concerns made by staff and document any treatment five in Nursing Notes
  • Obtain signed Health Care Provider forms, lab results, consultant reports and prescriptions. Implement physician orders. Inform pharmacy/Pharmacists of all new orders
  • Coordinate/implement a monthly delivery of clients medications by the Pharmacy
  • Attend psychotropic medication reviews as requested by the Service Coordinator or Case Manager
  • Maintain a safe, clean and healthy environment in accordance with L.I.F.E. policy and procedures
  • Complete Quality Assurance Nursing Assessments as required
  • Produce and type a variety of correspondence, reports, manuals and documents
  • Ensure adherence to federal, state and regulatory guidelines and accreditation standards
  • Ensure compliance with policies and procedures of L.I.F.E.
  • Liaison to families and individual of those being served by L.I.F.E.
  • Liaison to other service agencies being utilized by L.I.F.E.
  • Other duties as assigned
