Responsibilities Provide comprehensive nursing assessments, including vital signs, physical examinations, and patient history documentation using Electronic Health Record (EHR) systems. Administer medications, injections, IV infusions, and perform catheter care following strict protocols to ensure patient safety and comfort. Develop and implement tailored care plans for patients with various needs such as dementia, Alzheimer's disease, chronic illnesses, or post-acute recovery. Educate patients and their families on disease management, medication usage, nutrition, and health coaching to promote optimal health outcomes. Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to community resources or specialty services like hospice or physical therapy. Conduct patient assessments during home visits, monitor symptoms of acute pain or infectious diseases, and triage urgent concerns with professionalism and urgency. Maintain accurate medical documentation adhering to HIPAA regulations while utilizing EMR/EHR systems such as Epic or Cerner for seamless record keeping. Skills Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Bilingual proficiency in [specify languages], enabling effective communication with diverse patient populations. Critical care experience including ICU or emergency medicine settings is highly desirable. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and medical documentation standards. Proficiency in IV insertion, medication administration, sterile technique, and infection control practices.
