Job Description
Areas of Responsibility A. Provide Direct Care to Residents Provide direct resident care as needed. Administer medications and treatments according to policy and procedures. Implement safety, infection control, and preventative measures; use proper aseptic techniques. Deliver resident care according to the plan of care. B. Coordinate, Supervise, and Evaluate Resident Care Supervise nursing assistants and other direct care staff. Visit and evaluate residents according to assignment to monitor condition and response to treatment. Ensure goals and interventions from the plan of care are followed; communicate via verbal instructions and CNA Kardex. Initiate, update, and implement plans of care as required. Observe staff-administered care to ensure safety, effectiveness, and timely completion of duties (e.g., BM record, daily intake forms, PRN medication logs, MDS grids). Supervise and ensure completion of daily, MDS, and Q-shift documentation; follow up on labs, new orders, and other documentation. Contribute topics for in-service training sessions. Schedule physician appointments and arrange transportation as needed. Recognize staff performance and conduct in line with facility policy to support retention of qualified personnel. Accompany physicians during resident rounds, assisting with exams and documentation. Complete admission and discharge processes, ensuring all documentation and assessments meet regulatory requirements. Participate in Quality Assurance meetings as requested. Perform tasks outlined in the Staff Nurse Orientation Checklist. Attend and participate in initial orientation and regularly scheduled in-services. Attend and participate in mandatory in-services as assigned. Minimum Qualifications Education Graduate of an accredited or state-approved School of Nursing. Current license to practice as a Licensed Practical Nurse (LPN) in the State of Indiana. Knowledge and Abilities Positive attitude toward working with the elderly. Flexible and tactful in planning care to maximize resident independence. Able to work under varying degrees of supervision while maintaining open communication with the Director of Nursing. Able to assess resident health status, plan, implement, and evaluate nursing care, contributing to the resident's care plan. Able to organize nursing care and communicate effectively with physicians, pharmacists, and other staff regarding resident needs and orders. Knowledgeable of facility policies and procedures to ensure compliance with State regulations. Physical Requirements Stand, walk, and sit throughout the length of scheduled shifts. Use hands for repetitive grasping, pushing/pulling, and fine manipulation. Frequently bend, squat, and lift during the shift. Occasionally lift 75-100 lbs and frequently lift 35-50 lbs; frequent standing/walking, occasional sitting. May require two-person lifts/transfers or use of mechanical lifts.
