Monday to Friday, 8:00 AM to 5:00 PM, minimal on-call Position Overview Redcrest Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled in-home patient care. Compensation and Benefits Guaranteed hourly rate plus uncapped bonus opportunities Mileage reimbursement and additional incentives Medical, dental, and vision insurance options Paid time off, 401(k), and company-paid life insurance Employee Assistance Program and team support Qualifications Active LPN license in Indiana At least one year of acute care experience or home health experience preferred New grads are welcome to apply Valid driver's license and auto insurance Key Responsibilities Provide in-home nursing care following physician and RN direction Monitor patient conditions and report changes Administer medications and educate patients and families Maintain accurate, timely clinical documentation Participate in team meetings and on-call rotation Essential Functions Travel to patient homes within assigned territory Complete scheduled visits consistently and on time Document visits within 24 hours per guidelines Follow all state, federal, and company regulations Deliver care within LPN scope of practice
