Week Description:
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a Long-Term Care setting. The LPN will administer medications, perform treatments, monitor resident conditions, and collaborate with the healthcare team to ensure the highest level of resident care and safety.
Responsibilities:
Provide direct nursing care to long-term care residents. Administer medications and treatments as prescribed. Monitor, assess, and document resident conditions and report changes to the provider or RN. Perform wound care, medication passes, and other nursing procedures within scope of practice. Assist with admissions, discharges, and care planning. Maintain accurate and timely documentation in the electronic medical record (EMR). Educate residents and families regarding care plans and treatments. Ensure compliance with facility policies, infection control standards, and regulatory requirements.

1+ Years of LTC, M/S or SNF Experience•[

LPN License Pay:
$37.00•$43.00 per hour Application Question(s): Do you have at least 1+ year of LTC/Med-Surg/SNF experience? Do you have experience using Epic EMR? Do you currently hold an active AHA BLS (American Heart Association Basic Life Support) certification? Do you hold an active LPN/LVN license in the state of practice?
