Role:
We are seeking a compassionate and experienced LPN to join our growing team in the Great Bend area. This is a rewarding opportunity to provide one-on-one care in the comfort of patients homes while enjoying flexibility and autonomy in your schedule.

Key Responsibilities:
Deliver skilled nursing care to patients in their homes Develop and implement individualized care plans Monitor patient progress and communicate with physicians and families Educate patients and caregivers on disease management and home care Maintain accurate and timely clinical documentation
Qualifications:
Active LPN license in Kansas or compact license Current BLS certification Minimum 1 year of nursing experience (home health preferred) Valid driver's license and reliable transportation Strong communication and critical thinking skills Compassionate, self-motivated, and patient-focused Apply Today!
