PT/PRN LPN/RN
Job Summary:
Riverstone Post Acute, a leading healthcare provider, is seeking a dedicated and compassionate PT or
PRN LPN/RN
to join our team in Salina, KS. As a PT or PRN LPN/RN, you will play a vital role in providing high-quality patient care to our residents. We take pride in fostering a collaborative and supportive environment, and we are excited to welcome a talented healthcare professional like you to our family.
Responsibilities:
Provide skilled nursing care to residents, including medication administration, wound care, and vital sign monitoring Develop and implement individualized care plans in collaboration with the interdisciplinary team Maintain accurate and up-to-date records of resident care and services Collaborate with other healthcare professionals to ensure seamless care transitions Participate in Quality Improvement initiatives to enhance resident outcomes and satisfaction Maintain confidentiality and adhere to HIPAA guidelines
Requirements:
Current LPN/RN license in the state of Kansas 0 minimum years of experience Ability to work variable shifts, including evenings, weekends, and holidays Strong communication and interpersonal skills Compassionate and empathetic approach to patient care
