LPN Nursing Full Job Description Job Overview:
LPN Clinical Support Aquinas Home Health is seeking a detail-oriented and dedicated Licensed Practical Nurse (LPN) for a Clinical Support position. The ideal candidate will play a vital role in supporting our clinical operations by reviewing Epic referrals from The University of Kansas Health System (TUKHS), inputting F2F (Follow-up) dates with accuracy, and ensuring timely follow-up on labs sent to the office. The role also includes reviewing new orders and accurately inputting them into MatrixCare to ensure proper documentation and seamless patient care.Key Responsibilities:
Epic Referral Review :
Review Epic referrals from TUKHS to ensure that patient information and clinical requirements are accurately documented for further action. F2F Date Entry :
Input Follow-up (F2F) dates into the system, ensuring all dates are accurate and aligned with clinical workflows.Lab Follow-up :
Track and follow up on labs sent to the office, ensuring results are received and communicated to appropriate clinical staff for review and action.New Order Review & Input :
Review new medical orders, ensuring accuracy, and input them into MatrixCare for proper tracking and follow-up by the healthcare team.Collaboration :
Work closely with physicians, nurses, and other clinical staff to ensure seamless patient care and ensure proper documentation across systems. Attention toDetail :
Maintain a high level of accuracy in data entry and clinical record-keeping to ensure patient safety and compliance with healthcare regulations.Qualifications:
Valid LPN license in the states of KS and MO. Strong familiarity with Epic and MatrixCare systems, or the ability to quickly learn and navigate new software. Excellent organizational skills with the ability to handle multiple tasks efficiently. Strong communication skills for collaboration with clinical staff and timely follow-ups. Previous experience in a clinical support or administrative role is preferred.
