Work Schedule:
Monday-Friday 8:00am-5:00pm, 40 hours per week. Under direction of RN supervisor:
Provide direct and indirect services to patients. Provide medication for patients through pharmacy refills, samples, PAP, or other means. Utilize electronic prescribing system. Respond to patient inquiries/issues for medication refills by telephone and communicate with medical provider. Document verbal orders from medical provider. Document all patient interactions. Assist medical providers in patient care or coordination. Assist with patient care in the Injection Clinic. Regular, reliable, and non-disruptive attendance is an essential job duty, as is the ability to create and maintain collegial, harmonious working relationships with others. Employee must be able to complete complex tasks by applying CCBHC practices, standard office policies, authorized instructions, and past precedents to achieve a desired outcome. Employee must quickly recognize emergency or sensitive situations and take appropriate action. Critical decision-making and the ability to think through the consequences of a decision are essential in this position. Assist patients to obtain medication and encourage medication compliance. Reviews requests for refills via fax and ePrescribe. Review medical orders. Per provider's orders, complete refills utilizing ePrescribe. Monitor patient compliance of medications and report concerns to appropriate team members. Monitor compliance with attending scheduled appointments. Monitor signs and symptoms of medication side effects and communicate to provider.
Assist patient in obtaining medications:
i.e.… planners, refills, PAP/payer sources/vouchers, or sample medications. Assist in the Injection Clinic. Review medical orders for injectable medication. Prepare clinic for the day- get meds ready, review patient list. Administer injectable medications per provider orders. Document administration of medication. Complete billing in a timely manner. Coordinate patient care. Work medical provider orders as directed. Contact with pharmacies to coordinate or ascertain appropriateness of refills. Utilize electronic prescribing system, or other means, to refill medication.
Coordinate care with outside agencies:
group homes, substance abuse treatment centers, nursing homes, medical offices, hospitals, etc. Communicate, as needed, with family members, medical providers, case managers, therapists, or other care partners, including the Community Crisis Center.
Minimum Qualifications:
One year of experience as a Licensed Practical Nurse. Graduation from accredited school of practical nursing. Licensed Practical Nurse in the State of Kansas. Per Sedgwick County policy, this is a driving level position that requires a valid US driver's license without restrictions and current proof of automobile insurance. CMHC/CCBHC
licensing standards and pass
KBI, DCF
child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
Preferred Qualifications:
One year of work experience in the mental health or substance use treatment fields. Applicants have rights under Federal Employment Laws.
