We're looking for an experienced LPN leader who can balance clinical oversight with team development while helping create an engaging, person-centered environment for our residents—especially within Memory Care. If you're someone who leads with empathy, thrives in a fast-paced environment, and enjoys supporting both residents and staff, this could be the perfect next step. What You'll Own In this dual leadership role, you'll support overall wellness operations while directly overseeing the Memory Care neighborhood. Help lead daily clinical operations alongside the Wellness Director Provide leadership and support to caregivers, med techs, and nursing staff Oversee Memory Care programming and ensure residents receive individualized, meaningful care Complete resident assessments and assist with care plan development and updates Monitor changes in resident conditions and respond with strong clinical judgment Build strong relationships with families and serve as a trusted resource Support hiring, coaching, training, and accountability for team members Maintain compliance with state regulations, documentation standards, and survey readiness Collaborate with department leaders to ensure a seamless resident experience What You Bring to the Table Experience in Assisted Living, Memory Care, or Long-Term Care Active LPN license in Kentucky Prior leadership or supervisory experience preferred Strong understanding of dementia care and resident-centered approaches Ability to lead with professionalism, compassion, and accountability Strong organizational and communication skills A hands-on leadership style with a willingness to jump in and support the team The Details Full-time salaried leadership position On-call rotation and Manager-on-Duty responsibilities Active presence in the community—this is not a desk-only role Opportunity for growth with a supportive senior living organization Let's Connect If you're ready to join a community where your leadership truly makes a difference, we'd love to hear from you.
